WASHINGTON - Military commanders will be told to identify the troops in transgender or gender-irritated units and then send them for medical checks to force them out of service.
A senior defense official elaborated on a directive aimed at fulfilling President Donald Trump’s instructions to evacuate trans service personnel from the U.S. military.
The commander’s new order relies on regular annual health checks that require service members to perform. Another defense official said the Department has now canceled plans to pass the troops' health records to identify people with gender irritability.
Instead, trans units that are unwilling to stand up may be made by commanders or other soldiers aware of their medical conditions. Gender irritability occurs when a person's biological gender does not match gender identity.
Defense officials spoke on anonymous condition to provide details of the new policy. The process is compared with the early “Don’t Ask, Don’t Say” policy, which sometimes had commanders or other military members outside the outskirts of the troops who were not allowed to serve publicly at the time.
Active-duty troops must voluntarily identify themselves with the Ministry of Defense by June 6, and National Guard troops must be retained by July 7.
The sector provides financial incentives to those who leave voluntarily. They will receive twice as much as the separation salary that does not stand up.
Initially, officials said the Department of Defense would begin to identify anyone who did not voluntarily propose it through medical records. The details are not included in the new guide released Thursday.
Defense officials said that although the department believes it has the authority to review medical records, it would rather go through a more routine health assessment process. Traditionally, all service members have annual health assessments to determine whether they can still serve in medical care.
A new question about gender irritability is being added to this assessment. Active duty military personnel who do not volunteer to stand up must acknowledge their gender irritability during this medical checkup, which may be months from now on.
Unit commanders can speed up health assessments.
Under the new policy, “Commanders who are aware of the history of gender dysphoria, or symptoms consistent with gender dysphoria will direct personalized medical records reviews of such service members to confirm compliance with medical standards.”
Defense officials said it was the responsibility of service personnel and commanders to comply with the new procedures. Senior defense officials said the department is confident and comfortable with commanders who implement the policy and it does not think they will use the program to retaliate against service members.
The Supreme Court recently ruled that the Trump administration could enforce bans on trans people in the military, while other legal challenges took place. Three liberal judges in the court said they would suspend the policy.
Officials said 4,240 soldiers were in active service as of December 9, 2024, and gender irritability was diagnosed in the National Guard and reserves. But they admit that the number may be higher.
The total number is about 2.1 million troops.
Pentagon spokesman Sean Parnell said in a statement that earlier this month said about 1,000 troops had identified themselves and that they "will start a voluntary separation process." It can usually take several weeks.
Trump attempted to ban trans forces during his first term while allowing those currently serving to move forward. Then-President Joe Biden overturned the ban.
The new policy is not among those currently serving and only limited exemptions or exceptions are allowed.
Trump and Defense Secretary Pete Hegseth claimed that gender-irritated troops did not meet military standards. Hegseth tied the opposition to a campaign to get rid of the “suffocation” department.
